Wisconsin Dan Shansky M 2012-06 Recall Ineligible district voting 1 Dan Shansky left Wisconsin in March 2012 to take a job with a union in California, but still cast a ballot in Wisconsin's June 5, 2012 recall election. The community organizer, who listed the Milwaukee-based Community Action Now as a recent employer, was heavily involved in the protest and recall movement in Wisconsin. The government roll shows Shansky applying for absentee ballots for both the recall’s primary and general elections. Connecticut Andrew N. Patrick M 2008-11 Presidential Multiple Voting The City of New Haven Registrar of Voters brought a complaint against Andrew N. Patrick alleging that he voted twice in the 2008 Presidential election. The Connecticut Elections Enforcement Commission investigation concluded that the matter should be dismissed. Ohio Ohio Organizing Collaborative 2015 Registration fraud Forgery In October 2015, the Ohio Secretary of State's office received reports from county boards of elections about suspicious voter registration forms submitted by employees of the Ohio Organizing Collaborative. Boards have described significant irregularities among the applications, including voter registration forms from persons who are deceased, voter registration forms with multiple names, address or identification mismatches, and voter registration update forms that appear to have been signed by someone other than the voter.
