Hawaii Eight unnamed individuals 2010 Multiple voting 8 Hawaii County Clerk Jamae Kawauchi discovered several dozen Hawaii Island voters with duplicate voter registrations and found four people who voted twice in the same elections in 2010. Kawauchi said the clerk’s office in Hilo also discovered records showing four people who were allowed to vote twice in the 2010 elections in different districts and precincts. State officials notified the FBI, which began gathering information on the case but never opened a formal investigation.

West Virginia Dallas Toler M 2012-04 Primary 2013-10 Registration fraud - ineligible felon 1 0 Dallas Toler was charged in federal court with one count of voter registration fraud for falsifing a voter application for the 2012 primary election while serving as Mingo County Chief Magistrate. Toler submitted an application for someone who he knew was on probation for a felony and not eligible to register or vote.
Oregon Denise Michele Lyman F 2005-01 Petition Fraud 3 Denise Michele Lyman allegedly forged signatures on a petition for Ralph Nader for president. Charged with 3 counts of making false statements under election laws, a Class C felony. 36 months probation, 80 hours community service and barred from collecting signatures.
Kentucky Henry County - Name Unknown 2005 Registration Fraud An unnamed individual was charged with destruction of registration records. Charge was dismissed.
Florida Shalonda Michaelle Brinson "Madison 9" F 2010 Madison school board 2011-11 Absentee/Mail 10 9 Shalonda Michaelle Brinson, one of the "Madison 9," was charged with nine counts of felony fraud in connection with casting a vote, and one count of provided a false report to law enforcement authorities, as part of a scheme to illegally request, distribute, and collect absentee ballots in the 2010 Madison school board election. Mississippi Connie Hollins F 2007-08 Democratic Primary 2008-08 Absentee Ballot Fraud Connie Hollins was accused of voter fraud involving absentee ballots during the August 2007 Democratic Primary in Wilkinson County. Found not guilty at trial.
Kentucky Corey Page Jeffrey Newport Martha Hughes Tony Gumm William Proffitt Michael Page M 2006-11 General Vote buying Corey Page pleaded* guilty to conspiring to buy votes in the 2006 Monroe County general election, along with Jeffrey Newport, Tony Gumm, William Proffitt, Martha Hughes, and Michael Page. From October to November 7, 2006, the group conspired to pay voters to cast walk-in absentee ballots. *Plea deal: 2 years probation, 6 months home detention, $2000 fine
