Indiana Arthur Vera M 2003 Democratic primary D 2005-10 Ineligible district voting 1 Arthur Vera pleaded* guilty to voting in a precinct other than his own in the May 2003 Democratic primary, in which longtime East Chicago Mayor Robert Pastrick narrowly defeated challenger George Pabey. The Indiana Supreme Court overturned Pastrick's victory due to widespread voter fraud and ordered a new election.

Pennsylvania Ashley L. Clarke F 2009-05 Registration Fraud While working for ACORN, Ashley L. Clarke was charged with forgery, solicitation of registration for pay, unsworn falsification of authorities, obstructing the administration of law or government function, interference with voter registration, and impersonating another person in a voter registration application.
Florida Astrid Natalia Torres-Perez F Non-citizen voting Astrid Natalia Torres-Perez was one of 15 noncitizens charged with voting in various elections. Pleaded. (United States v. Torres-Perez; 2:04-CR-14046) 1 year of probation
Illinois Audrey R. Cook F 2016-11 General R 2016-11 Absentee/Mail Perjury 2 1 Audrey R. Cook, a long-time election judge, pleaded not guilty in Madison County Circuit Court to two counts of felony election fraud after admitting to submitting a mail ballot for her recently-deceased husband Vic Cook. Cook faces two Class 3 felony charges, one relating to sending in a false ballot, and one for violation of the absentee ballot statute. She is due to appear in court in February 2017.
Connecticut Audrey Schulde F Absentee/Mail Illegal Assistance Schulde was accused of neglecting to provide information about herself required by law, such as her signature, when she assisted about eight residents of a convalescent home with their absentee ballot applications. Civil penalty of $100.00. Ordered to comply with the law in the future.
Washington Augustin Park M 1 Records received from Snohomish County Washington did not indicate the specific accusation. From a list of almost 200 voter fraud cases that occurred since June 2007, only one person was charged. According to the prosecutor's office, the remainder of the cases were not prosecuted and no records were kept on the type of specific charge for the unprosecuted cases.
