Connecticut Michael Abbatello M 2004-11 Multiple Voting Michael Abbatello was accused of voting more than once in the November, 2004 election. He registered in both West Hartford and Old Saybrook, and had been registered in both towns since 1985 because he did not provide the Old Saybrook registrars with his prior voting address. He voted in Old Saybrook, but did not vote in West Hartford in the presidential election. He has been removed from the list in West Hartford. Dismissed

Indiana Dwaine Lay M 2003 Democratic Mayoral Primary Perjury 2 Dwaine Lay was charged as part of a fraud case brought by the Lake County Joint Voter Task Force that ultimately overturned the result of the 2003 East Chicago Democratic mayoral primary. Texas James Alan Jenkins M 2010-05 Woodlands Road Utility District 2012-03 Ineligible district voting Registration Fraud James Alan Jenkins was one of seven voters indicted for voting outside their district. The group planned to vote in a district where they did not reside in order to elect three of their members to the board of directors of The Woodlands Road Utility District. The group illegally registered in the district by using the address of a hotel in the district as their home address of their voter registration application. Washington Dawn Johnson F 1 Records received from Snohomish County Washington did not indicate the specific accusation. From a list of almost 200 voter fraud cases that occurred since June 2007, only one person was charged. Connecticut Brenda L. Schlemmer F 2009-11 Absentee/Mail Forgery An investigation by the Connecticut State Elections Enforcement Commission determined that Schlemmer obtained and completed a single absentee ballot application with the Danbury Town Clerk by falsely signing her daughter's name to the absentee ballot application under penalty of false statement and thus obtained a single absentee ballot, which she did not cast. Schlemmer admitted guilt. Received civil penalties totaling $2,000 and ordered to comply with the law in the future, and not to handle or process absentee ballot applications for five years.
