California 113 Contra Costa County voters 2016-06 Primary Multiple voting 113 The Contra Costa County Elections Division discovered 113 registered voters voted twice and had both ballots counted during the June 2016 Presidential Primary Election, as a result of an emergency accommodation to the California Secretary of State’s office. Connecticut Mamie Jackson M Absentee/Mail Illegal Assistance Jackson, a campaign volunteer, was accused of not signing absentee ballot applications she assisted with, as required by law. She was also accused of being present while people voted absentee ballots, a violation of state laws, because she was an agent of a political party. Civil penalty of $5,000, reduced to $500 due to financial hardship. Ordered not to assist with absentee ballot applications or distribute absentee ballots for two years. Ordered not to be present when people are filling out absentee ballots or to take them and to comply with the law in the future.

Ohio Daniel "Tate" Hausman Amy Little Yolanda Hippensteele M 2008 General D Ineligible district voting Absentee/Mail 1 1 Daniel "Tate" Hausman (New York native, staff member for Vote Today Ohio, and out-of-state campaigner for Barak Obama) was one of several people who came to Franklin County, Ohio, registered to vote after 30 days and voted, eventhough he did not intend to remain a resident of the county and state. He and his co-defendants chose to vote in Ohio over their home states, because Ohio is a swing state and Obama was expected to easily win in their home states. Charged with a violation under Title 35 of Ohio Code, casting an absentee ballot despite not meeting residency requirement. Pleaded guilty.
