Connecticut David Ford M 2004-11 Presidential Ineligible felon voting David Ford was accused of applying for a ballot and voting in the 2004 presidential election even though he was a felon whose voting rights had been revoked. Ford's attorney provided a copy of the notice of his rights restoration, which indicated that Ford was no longer under the jurisdiction of the Connecticut Department of Correction and that his discharge date was July 30, 2004. Ford was eligible to have his voting rights restored on July 30, 2004. No further action was taken on complaint. Dismissed
California Ricardo Lopez-Munguia M 2008-11 General 2012 Non-citizen voting 3 1 Ricardo Lopez-Munguia, a Mexican who was deported decades ago for drug trafficking, pleaded* guilty to living illegally in Escondido under a false identity, making a false claim to U.S. citizenship, and fraudulently voting in the 2008 U.S. presidential election. Lopez-Munguia assumed the identity of Gustavo Carranza-Madrigal, a U.S. citizen, and obtained a fraudulent California driver's license, a Social Security card and a U.S. passport. Iowa Unnamed 2016-10 General Multiple voting 1 1 Three people have tried to vote multiple times in the November 2016 general election in Polk County, according to Des Moines police reports. Two of the incidents involved people casting mail-in ballots and also voting in person at one of the county's early voting locations. Indiana Monte Murphy M 2007 Muncie municipal D 2009-05 Absentee/Mail Illegal assistance 9 3 Monte Murphy, a former Muncie City Councilman, was convicted of 3 felony counts of illegally receiving absentee ballots in the 2007 Muncie municipal election. Murphy provided ballots to the individuals, told them how to vote for a straight Democratic ticket, and then mailed in the ballots for them. New York Elbin Lopez Elbin Lopez Betty Julien Luis Vargas (campaign workers for Democrat Bronx Councilwoman Maria Del Carmen Arroyo) M 2013-09 Bronx Assembly special - Democrat primary D 2014-03 Petition fraud Forgery Elbin Lopez, Betty Julien, and Luis Vargas - former campaign workers for Bronx Councilwoman Maria Del Carmen Arroyo's 2013 reelection bid - were charged with forging nearly 100 signatures on the candidate's primary ballot petitions, including celebrities' names as well as fake addresses, according to the Bronx District Attorney's Office. The three were charged with multiple felony counts, including falsifying business records and forgery. Washington Gordon Hill M 1 Records received from Snohomish County Washington did not indicate the specific accusation. From a list of almost 200 voter fraud cases that occurred since June 2007, only one person was charged.
