Missouri Danny Lawrence Williams Keven Hayes Tracy Renee Jones Rogell Coker Jr M 2014 General - 2014 (Initiative) 2015-05 Petition fraud Forgery 0 Felony forgery warrants were issued for Danny Lawrence Williams, Tracy Renee Jones, and Rogell Coker Jr. Keven Hayes was also arrested in Columbia on three felony counts of forgery over fraudulent signatures on a 2014 initiative petition that unsuccessfully sought to put an early-voting proposal on the Missouri ballot. Secretary of State Jason Kander turned its investigative findings over to Boone County law enforcement officials, who filed the charges. There were 1,000 questionable signatures flagged in Boone County alone, including those of deceased individuals. Investigators allege that Hayes submitted petitions with 363 signatures from Boone County, including names of deceased individuals and three people who when contacted said they hadn't signed the petition. Ohio ? M 2008 2010 General Multiple voting Cross-state voting 2 On March 23, 2012, the Ohio Secretary of State said a Fulton County man may have been registered and voted in Ohio since 2006 and North Carolina since 2002, and may have voted in both states in the 2008 and 2010 general elections. Voter registrations in both states have the same name and personal identifying information. Connecticut Lori Pfau F 2009-04 Referendum D Absentee Ballot Fraud A complaint was brought against Lori Pfau alleging that she mailed absentee ballot applications without including warnings and eligibility information, as required by law, and that she did not first register with the town clerk before taking such action, among other charges. North Carolina Unnamed Poll Worker 2010-11 General Impersonation In person 1 According to the Lenoir County Board of Elections minutes, during the 2010 general election a poll worker pulled the master list, removed the necessary label, grabbed a ballot and proceeded to vote in place of a member of the family. Details as to the worker, the precinct and the candidate to whom those votes would have gone have not been released. Former LCBOE Chairman Robert Waller said the board was informed of the incident at its Nov. 12, 2010, meeting, and sent the case on to the State Board of Elections. From there, an SBOE investigator pursued the case, taking statements from elections director Dana King and other poll workers. “It’s my understanding that that matter was referred to the D.A. for your prosecutorial district,” SBOE Elections Liaison Veronica Degraffenreid said.
