Pennsylvania FieldWorks employee(s) 2016 General D Registration fraud Pennsylvania state police raided the Delaware County office of FieldWorks LLC seeking evidence of possible voter-registration fraud. In a warrant filed in October 2016 in County Court, investigators said they were seeking documents, financial information, and lists of employees at the Norwood office of FieldWorks LLC, a national organization that does political work for Democrats. Washington Allen Maberry M 1 Records received from Snohomish County Washington did not indicate the specific accusation. From a list of almost 200 voter fraud cases that occurred since June 2007, only one person was charged. Oregon Johnny R. Burkett M 2006-02 Forgery 5 Johnny R. Burkett was charged with submitting forged signatures. Pleaded guilty to 2 of 5 counts of making a false statement under election laws, a Class C felony. The other 3 charges were dropped after plea negotiations. 10 days in jail, 36 months of probation, a $750 fine (suspended) and barred from collecting signatures during probation.

Tennessee Unknown 2012-11 General In person Impersonation Multiple voting DeSoto County Election Commission chairman Paul Beale said several voting irregularities in the Nov. 6, 2012 general election and Nov. 27 runoff were turned over to the proper authorities, including the FBI. District 4 Election Commissioner Carl Payne reported an incident in which a father cast an absentee ballot, the son voted in person and then the son changes clothes and returned to vote as his father. according to a written statement from the poll manager. Among other cases reported by Payne: A voter came to cast a ballot, gave a name that was on the poll book, signed the receipt book and was allowed to vote. Another person using the same name came to vote later that day, and was informed he'd already voted. The second person provided proof of identity.
